The following professionals are likely to be involved in your healthcare. Your General Practitioner, Consultant Gastroenterologist, Dietitian/Nutritionist and your Pharmacist.
How to communicate effectively with this team
When you have been diagnosed with Crohn’s disease or colitis, you will need to communicate with your healthcare team, to ensure you are getting the best possible treatment.
Have all their telephone numbers within easy reach so you can contact them whenever you need.
In between your visits, note any changes that seem significant to you e.g. weight loss/changes in bowel habits, tiredness, lethargy and reactions to certain foods. Bring the information with you to your next visit and discuss it. No information is insignificant.
Express any concerns you may have, as you may find there is a very simple solution ready at hand for you.
Don’t forget to talk to your pharmacist about your medication and ensure you understand what it is, why you are taking it, and what potential side effects to watch out for. Your pharmacist will only be too willing to answer any questions you may have.
If you forget to ask a question or explain something at your visit that you feel is important, just telephone the relevant professional and your query will be dealt with.
