Swollen nipple - cause in teenage male?

My 13-year-old son has a swelling on his right nipple, which is not painful. What might cause this?

This is a very common finding and can happen in as many as 50% of adolescent males. It can be due to fatty tissue behind the nipple or possibly enlargement of male breast tissue. The two conditions can be distinguished from each other by gently grasping the swelling between thumb and forefinger. Breast tissue will be adherent to the back of the nipple whereas fatty tissue will be more mobile and not firmly attached to anything.
